spg = 400 for volume stack
spg = 0 for image stack
mz = 1 for image stack

Also offset data block by extended header length defined by nsymbt

r18372 modifies tomography tilt series mrc stack written in stack.php to include current standard change. Since stack.php result is of "AGAR' exttype, this stack would not be readable by ccp4 library.
