Edit user returns a blank form

Hello,
I'm running Appion/Leginon 2.2. If you login as the site administrator and go to Administration and select Users from the left side bar you see the table of users. However if you try to edit a user, say to correct their Department, you are presented with a blank form. In the address bar it is myamiweb/adduser.php?userid=7.

Unfortunately, I can not reproduce the error here. A blank form may be related to some error detected. You may want to turn on error_display in php.ini to see if it gives you any clue.
