Image Assessor is not working

I think that I have successfully installed Leginon and Appion, and updated to version 2.1.3. (CentOS 5)
Leginon data collection, and Appion 'DoG picking' and 'Auto Align Tilt Pairs' are working reasonably well.
However, for both 'Web Image Assessment' and 'Multi Image Assessment' there is a problem. The image loads, but when any of the buttons are clicked (blue arrows or keep or reject buttons) the screen seems to refresh, but does not advance to the next image, and assessment status does not update.
Also, for autotiltrun results there are two sets of images presented for each actual image pair (e.g. Image 1 and Image 2 both show the same image pair and picks, Image 3 and Image 4 show the same image pair and picks, etc.). It looks like this is because it is showing each image pair twice, first with the image name/number of the tilted image (00), and then with the image name/number of the untilted image (01). This is not a big problem, but it is confusing and inefficient, and seems unnecessary.
Please advise on how to troubleshoot, especially the image advance and status update issue. I would really like to be able to use these Image Assessor features.

Hi Brian,
The issue that you are having with the Image Assesment tools could be caused by incorrect database permission settings.
Please ensure that you have followed instructions found here: http://emg.nysbc.org/projects/appion/wiki/Additional_Database_Server_Setup_After_Web_Server_Installation
These instructions were added recently and may not have been available during your installation.
For the autotiltrun issue, were these images collected with Leginon, or were they manually uploaded?

It does seem likely that the problem has to do with the database permission settings. I found that the Image Assessor is functional when I access the web server using a computer connected directly to the internet at Stanford. There is only a problem when accessing the web server from a computer that is connected to the internet from outside Stanford, even though I am using a VPN and it should appear to be on the Stanford domain. So I need to set up permissions so that it will work through the VPN.
I was aware of the "Additional_Database_Server_Setup_After_Web_Server_Installation" instructions, but thought that Option 2 applied to my setup:
"Option 2: Less secure - Allow all privileges
If you are allowing all privileges, no further steps need to be taken."
Thus, I didn't think I needed to do anything.
So, today I followed the Option 3 instructions by typing at the mysql prompt:
GRANT DELETE ON leginondb.ViewerImageStatus TO usr_object@'%.stanford.edu';
GRANT DELETE ON projectdb.processingdb TO usr_object@'%.stanford.edu';
But this did not change anything, as far as I can tell. The Image Assessor still doesn't work when connected to the internet from outside Stanford, even with the VPN connection.
As for the autotiltrun issue, the images were collected with Leginon RCT.
