allow a 3D volume to be used as a mask in 3D refinement

Description
xmipp has this capacity. Eventually we could do this to other iteration based refinement, too.
r15765 add the preparation for xmipp single model projection matching. The input is and initial model id, which might not be appropriate. Made a separate database table apRefineMaskVolData for the prepared result. Not sure if this is the best way to do it, Alternative is to add a flag in apRefineInitialModelData and fix that later.
